Family Physician in Inner-city Ministry

His Branches, Inc., an inner-city Christian ministry that facilitates providing support and care to members of underserved communities in Rochester, NY, is seeking a full-time BC/BE Family Physician to join two others in an established practice of Christ-centered family care. Physicians working with the support of His Branches are asked to practice within the following guidelines:

1.   Submit to the Lordship of Jesus Christ while maintaining active, committed participation in a local Christian church under the spiritual oversight and guidance of its ordained leadership.

2.   Live in harmony with the His Branches Statement of Faith and within the Mission, Vision, and Foundational Principles of Grace Family Medicine as appended.

3.   Obtain and maintain credentials, licensure, hospital privileges within both Unity Health and Strong Health Systems in Rochester, and malpractice insurance coverage (with $1M, $3M minimum limits of liability) to provide clinical care commensurate with their training and experience in New York State and with all local health maintenance organizations and major insurance plans including both Medicaid and Medicare.

4.   Provide Christ-centered family medical and counseling services to any patient who seeks his clinical care through His Branches Inc. until his practice panel is full, regardless of any patient’s socioeconomic or insurance status or ability to pay, offering a sliding fee schedule to uninsured patients with incomes less than 200% of the household poverty level according to current Federal Poverty Guidelines. The provision of normal obstetrical care is optional but encouraged.

5.   Arrange for patients to be scheduled so that they can be seen at an average pace per 4-hour clinical session that is fast enough to be cost-effective (minimum 6 patients per session) but slow enough to allow quality time for an adequate assessment of each patient’s physical, psycho-social, and spiritual circumstances (maximum 12 patients per session).

6.   Develop and maintain a network of referral specialists and programs to manage conditions that exceed their training and expertise.

7.   Develop and maintain a comprehensive, continuous style of patient care that emphasizes health education, preventative health services and case management of chronic or complicated conditions, as well as crisis intervention and acute illness care.

8.   Participate in an on-call telephone coverage schedule with other similarly trained providers to assure that professional counsel is available after hours to patients enrolled for care at clinical offices maintained by or affiliated with His Branches Inc.

9.   Keep ongoing, complete, and accurate medical records of their patient care activities using an integrated electronic medical record and billing program in the office and participate in regular, peer-reviewed quality assurance analysis of their professional services.

10. Reimburse His Branches Inc. as billed each month for documented costs of support and space allocated to their practice by His Branches Inc. in accordance with an established cost-sharing plan.
