Enrolling as a New Patient

Thank you for considering becoming a patient in our practice. Before going any further, please call our office at 585-235-2250 to express your interest, find out if we are accepting new patients right now, and get things started. The following pages, listed below and accessed through the menu at the left, will lead you through the process of enrolling. We apologize in advance for the complexity of today's world and all the paperwork and information required. Please bear with us as we seek to provide you with the best possible care.

Welcome

This page provides some important information for you about our practice and how we can work together.

Narcotic Policy

It's the policy of Grace Family Medicine’s providers NOT to prescribe highly addictive pain medications, including Narcotics or other similar Controlled Substances, to patients who are new to the practice. This page explains our policy in detail.

Information About You

We need to know a lot about you and who to contact in case of an emergence, just to start a chart. Then we'll need to know all about your Health History, so we can provide the kind of quality care you deserve. This page has the forms that will guide you through the process of giving us what we need.

Financial Matters

We're ready to work with you to help you get the health care you need, regardless of your ability to pay. This page describes our financial policies and allow you to help us by telling us about your financial situation and any health insurance policies you may have.

Notice Of Privacy Practices

At Grace Family Medicine, we're committed to treating and using your personal health information responsibly. This section explains our Privacy Policies in detail and asks how you want to authorize us to share your health information.

Advance Directive Documents

The most difficult part about considering potential end-of-life decisions is thinking them through when you are relatively healthy and then communicating them to your loved ones. Once you have done this, obtaining and filling out the forms should be easy. Click here for further information and to create your advance directive documents online.
