Thanks for considering us as a partner to meet your health goals. This past year, we helped more than 3,000 patients receive quality care and were recognized by HRSA for being in the top 30% of clinics in the country. We look forward to caring for you as uniquely and specifically as possible as we have for our current patients.

We accept all people, regardless of insurance type. We have patients with commercial insurance (such as Excellus, MVP, Fidelus, etc.), Medicaid/Medicare, and no insurance. As our current patients can attest, we will never turn anyone away due to an inability to pay. We will never deny services based on a person’s race, color, gender, national origin, disability, religion, or sexual orientation.

What should I bring?

Please plan to arrive 15 minutes early for your first appointment. Please bring the following items from the list below:  

  • A Photo ID 

  • Insurance Card (for any type of insurance, including Medicare or Medicaid)

  • A list of any medicine that you take*

*Our team will not be able to prescribe or refill controlled medications on your first visit.

How do I make an appointment?

To make a New Patient Appointment, call us at 585.235.2250.  We will work to schedule an appointment convenient for you as well as help you know what to expect and answer any questions you might have. *

What if I don’t have insurance?

If you don’t have insurance, please bring proof of your household income (Items such as a recent check stub, a letter from your employer, IRS Tax Documents, W-2 or Unemployment Compensation Letter) as well as cash, check or credit card to pay for your visit.

What if I can’t afford my copay or have a high deductible?

You can apply for reduced copays or financial assistance for medical visits by providing proof of your household income and completing an application for our Sliding Fee Discount program. 

Our Case Manager is available to work with you in order to identify an affordable insurance plan for future visits. 


*The Sliding Fee Discount program is based on your income and family size. Your payment will be determined upon the completion and review of your application. Payments for service can range from a minimum of $5 to full responsibility. Click HERE to view our Sliding Fee Scale and Application.
