His Branches operates as a community health center with clinics in the 19th Ward and Beechwood communities. We are accepting new patients and offer Primary Care with Integrated Behavioral Health, in addition to providing the following services:
PRIMARY MEDICAL CARE
General primary care for the entire family. This includes: annual checkups, sick visits, screening and laboratory work, preventative care, well-child services, immunizations and women’s health services (routine GYN care, birth control methods, pregnancy testing and counseling, voluntary family planning, testing and treatment for sexually transmitted infections).
CASE MANAGEMENT
Services include assistance with obtaining insurance and access to medical transportation, financial assistance for medical visits and medication copays, linking to Medicaid services, food and housing information/assistance for individuals/families experiencing homelessness, and access to mental health services.
TELEHEALTH VISITS
IMMIGRATION HEALTH
Initial medical screening for newly arriving refugees is available for newly settled refugees, and Migrant Health Assessment provided by our Civil Surgeon to enable permanent residence. This service is available to all, regardless if you are a patient of His Branches. Additional primary care for ongoing health needs is also available.
Many appointments can be done remotely using an online video or phone conference method that provides an efficient way to attend appointments with our providers, from the comfort of your home or office. Telemedicine is appropriate for a wide range of services and you will receive the same quality of care as an in-person visit.
PEDIATRIC CARE
As a Family Medicine clinic, we provide annual well-child checks, sick visits, and vaccinations for children ages birth to adulthood as part of our continuity of care model.
MENTAL HEALTH COUNSELING
Non-invasive treatment leveraging SoftWave Technology, acoustic waves activate cellular receptors, initiating the body's natural biological healing response, effective for chronic pain and wound care. Treatments are available at our 19th Ward clinic for all, regardless if you are a patient of His Branches.
PAIN MANAGEMENT
& WOUND CARE
LAB SERVICES
For quicker results and quality care, we offer the convenience of an on-site lab which allows us to check diabetes, blood pressure and cholesterol while patients are in the clinic for their visit.
Medical care is provided by our clinical team directly to the Rochester residents that are unhoused in the areas surrounding His Branches at encampments and temporary shelters.
STREET MEDICINE
Through tailored conversation, our psychotherapists work with their patients to develop a personalized plan of treatment and counseling focused on the patient's needs for symptom relief, problem solutions or lifestyle changes. This service is available to all, regardless if you are a patient of His Branches.
LIFESTYLE MEDICINE
Making changes in nutrition, movement, sleep patterns, stress management and use of substances can improve the health status of a patient and reduce their risk of developing further conditions. Our Lifestyle Medicine specialist works with our patients to support them as they make all-important changes that lead to healthier lives.
